To participate in camp it is required each camper print the Medical Information Form and bring with you to camp!

The Medical Information Form must be signed by a parent or guardian as well as a physician's signature (or a copy of a signed physical dated less than one year prior to camp). In order for students to participate in camp activities, we must have this form by check-in the day of Camp.
